I'm pretty sure it started with a (dating sim) game called Katawa Shoujo. The main character, if I recall correctly, has a bad heart. All of the potential mates for the guy have disabilities, but they're just so gosh darned determined and cute. And it became a joke that when they do something really, really cute/endearing that it agrivates his condition, causing him to go "HNNNG" while clutching his chest.
