My idea is the Screwball-looking nurse is Rich's wife/DT's mom, Discord made his way through the hospital during his return to Ponyville, found the nurse too stuffy and boring, decided to fuse together the nurse with Screwloose and presto Screwball was born.
Screwball reverts to the two ponies when the magic healing wave hit. |

My fanon is Screwball is Diamond Tiara's aunt, and her mother is the Chubby pink Nurse Pony with the similar colour scheme. She works in the hospital to keep an eye on her sister. |
