Got to be a design flaw, or the guys who made it didn't understand it needed to move slowly at best.
If it's motorized, could be an electrical fault that caused it to go over the set limit. The Special School in my region had an automated merry-go-round, only did one revolution every minute, but when it had a fault, damn thing spun-out like a washing machine. |

I prefer to imagine there's a fluffy unicorn on the merry-go-round, spinning it with magic. It got scared when the thing started moving too fast, forgot it was the one spinning it, and couldn't stop. |
