Lemme break down the individual fetishistic elements shown in this picture
1: The most obvious being obesity. Here being shown with elements such as the double chin, large belly and large butt having been heavily exaggerated to a large degree. For some people bigger is better (mostly only when it comes to fictional characters) 2: Immobility. Immobility creates the sense that what happens to the character's body decided by those round them. A concept that is most prominent in practices such as bondage. 3: Flatulence. Again, a lack of control is implied here being such as unable to hold it back plus the humiliation associated with publicly farting. Other elements include being aroused by the smell of flatulence and farting along with just being considered dirty and being associated with the anus, which, has its own list of fetishes 4: Perspiration and salivating. Mostly the same as flatulence minus the humiliation and anal association plus the added bonus of being associated with heat and exertion 5: A clinical environment. Hospitals are often considered scary or uncomfortable by most people as, again, they give a sense that you're not entirely in control of what happens to you added to the fear that you might be ill or dying. What is nightmare fuel to some is fetish fuel for others. 5.5 The oxygen tube and heart monitor also add to the sense of a lack of control 6. There is a sprinkling of sadism in that twilight is clearly in discomfort and humiliated and the possibility that her current state is permanent or at the very least long term.
