I'm going to go out on a limb here and say that this is from Heavenly Turmoil, where Celestia fails to save a foal from a burning building. This failure is the final domino that sends her into a horrifying downward spiral. Like, good lord.
And there is no happy ending. But, I feel this limb is waaaaay out there. This might not have any context. |

There's also a fic where Discord's infected Celestia's mind, and makes her hallucinate, sets the fire in the first place, and kill the Wonderbolts. |
