I didn't actually think about it being painful first. I thought that its one of the toy ones without a needle but a funnel or so.

Who says that's blood? By the looks of it, that red liquid, whatever it may be (even if it's blood) was what came out of the syringe itself, as you can see some of it still inside the syringe. If that liquid is just dripping back out that that needle is either not sticking anything or isn't even there. |

Sorry, I don't look at the source. I was just using logic, that and I would much rather believe that the syringe is just being used as a douche instead as an injector. |

Additionally, fluid that remains in a syringe remains between the bottom (when needle is pointing down) and the disc that pushes it out. It makes no sense for any to be in that part of the syringe, so it's either an art error or the syringe is broken. |
