I think it's a Full Metal Alchemist thing (hence missing red jacket tag), but I still don't really get it. |

^^ It's a Chameleon Arch. Time Lords can rewrite their DNA to disguise themselves as another species through a very painful process. Their original DNA pattern is kept in a fob watch until it's opened. |

The watch is supposed to be able to contain the essence (or to be more frank, "soul") of a Timelord (which is what the Doctor is), leaving just enough behind for a specially-prepared body and mind.
The Doctor used it once to dodge the Family, who were tracking his Timelord-physiology. His prepared body was human, so the Family couldn't find him. The conclusion given here, is that instead of the animators being lazy and just giving a different cutie-mark on this pony (or they may be doing this on purpouse, I've spoted a lot of hourglass-cutiemarked ponies) the pony on the left was just a shell that later turned back into a Timelord-pony. |
