Way I figure it, the "royals" are families of ancient consorts, either descended by bastardy or the children of in-laws.
Of course, one of the other Princess might have just adopted a foal that was in difficult circumstances too. |

I imagine the setup in Equestria to be like in Exalted. Celestia/Luna ruled like the Scarlet Empress; a seemingly-immortal seized control of an ancient power to put down the rampaging forces of chaos, established a single great empire, and then took mortal consorts for company, elevating said consorts and their descendants into a royal bloodline. |

That's as good an explanation as any. Really, what is "noble blood" anyway? Just somebody who seized power and declared their family was more important than everybody else. |
