I think they didn't recognize her because JJ never draw a human Red Heart before. |

JJ sure knows how to draw. :)
Though I have to admit I recognised her only after a while. Nurse Redheart is quite difficult to humanize since she doesn't have so striking features as most ponies. None the less it is a very good pic. |
